At Buxton Healthcare, our Supported Living service helps adults live as independently as possible in their own homes or shared accommodation. We design flexible, person-centred support around each individual’s goals – from building daily living skills to accessing the community, education, and employment.
We work with adults (18+) who may be autistic, have a learning disability, mental health needs, physical disabilities, or a combination of needs. Whether you’re stepping down from residential care, moving on from the family home, or looking for a more independent alternative to supported housing, we can help plan a smooth transition.
Supported living separates housing from care. You choose where you live and who you live with; we provide the support you need to thrive. That can range from a few hours each week to 24/7 support, including sleep-ins or waking nights, depending on assessed need.
Your goals lead the plan. We build a personalised support plan with you (and those you choose), and review it regularly so it adapts as your independence grows.
We focus on outcomes that matter — improved wellbeing, greater choice and control, stronger daily living skills, and meaningful participation in the community.
Our staff are carefully matched to each person. They receive ongoing training in safeguarding, dignity in care, medication, autism awareness, learning disability, mental health, and de-escalation/positive behaviour support.
No. In supported living you hold your own tenancy (or share), giving you more choice and control over your home. Care and housing are arranged separately.
Yes. If you prefer shared living, we’ll help explore compatible housemates and properties. You can also live alone with support if that suits you best.
Your plan is reviewed regularly. Support can step up or down, including short-term increases (for example, after hospital discharge).
Where assessed and commissioned, our teams can provide dignified personal care as part of a broader independence-focused plan.
After assessment and agreement of funding, we work with you to create a safe, timely start - phased if needed to build confidence.
